| Description | Diabetes Education for Health Care Providers is an applied certificate program. It prepares health care providers with the knowledge, judgment, skills and attitudes needed to meet the health care needs of individuals with diabetes.
The program is for health care providers with the following designations: Community Health Representative (CHR); Community Health Educator (CHE); Continuing Care Assistant (CCA) and Primary Care Paramedic (PCP). | | |
